As well as using this form we give you the chance to give us feedback every time you log a servicedesk call, as you will automatically get a feedback survey link when each call is closed. If you've overlooked that email you can log into the servicedesk portal (helpdesk.uhi.ac.uk), and in the history section there will be an option to take the survey any closed calls.
